Before you come to our medical center
We ask you to fill out your medical history form and medication history form online now. When completed, you click “submit” and the form goes directly to
us! Again, please try to complete these forms a week in advance of being admitted

Valuable information: Also, please print off and read the new herbal and supplemental medication information. These products should be stopped 1 week prior to being admitted here. Click here for the form and details.
Special help: The Saint Elizabeth CARE office will also help you complete as much of your additional paperwork as possible
before coming to the medical center. A nurse or other trained healthcare professional
from the Saint Elizabeth CARE office will contact you before your scheduled surgery
or major procedure. They will also give you information on preparing for your
surgery or procedure and answer all your questions!
