New Hire Paperwork

Congratulations on your recent offer for employment at Saint Elizabeth. We hope to have you as a member of our team soon.  We want to remind you that your employment is still contingent upon you satisfactorily passing a background check and a pre-employment physical which includes a substance abuse screening.
 
STEP # 1: Advance paperwork-Human Resources documents
Please print off and fill out the paperwork listed under this paragraph. This will make your employment process proceed as smoothly as possible. Keep your eye on your e-mail or mailbox---you will receive vital information from us very soon. It will include additional information including the date of your mandatory New Associate Orientation. We will need your paperwork returned to us on or before your start date.
STEP # 2: Pre-Employment Physical
As part of our Saint Elizabeth employment process you MUST schedule a pre-employment physical at Company Care.  They are located at 5000 North 26th Street, Suite 200 - Lincoln, NE. The phone number is 402-475-6656. Keep in mind that most appointments take up to two hours. Also-please schedule your appointment so that it is completed a minimum of seven days prior to your start date.
 
Bring to Company Care:
You will need to print off and complete the following paperwork and take it with you to your appointment at Company Care. A map is attached for your convenience. For your appointment at Company Care:
Your completed paperwork (the forms below)
A current immunization record that includes 2 medically documented dates for Rubella, Rubeola, and Mumps (or MMR.) If you do not have this, we will do a blood draw to determine your immunization level.
Also-Do not smoke, chew gum or have hard candy in your mouth one hour prior to your appointment as you may need a mask fit test.  You may eat and drink.
Please understand that your employment could be delayed or terminated by any of the following reasons:
  • Your physical was not completed a minimum of 7 days prior to your start date
  • You did not pass your physical or substance abuse screening
  • You did not pass your background check
  • You did not complete your paperwork and all necessary documentation
